While we're on the subject, how are piano reductions done? I mean that an orchestra might have 20 diff. parts playing at the same time. How do you know which ones to put in?

You have to choose the most oimportant parts as well try keeping the correct overall harmony. It's a long work but not difficult for who knows harmony.
